- Setup scripts, README and CLAUDE.md still clone and reference the retired caura-memclaw repo name
- memclaw-governance.md is committed four times and overwritten by setup — pick one source of truth
- setup.ps1 reports 'Registered' even when 'openclaw agents add' fails (native exit code not checked)
- setup.sh discards stderr from 'openclaw agents add' and labels every failure 'may already be registered'
- Setup scripts hardcode http://localhost:8000 and ignore MEMCLAW_API_URL
- README tells macOS/Linux users to run 'setup.sh', which isn't executable or on PATH
- SECURITY.md asks for private reports but names no contact or channel
- README table of contents links to a dead anchor (#how-it-works)
- MEMCLAW_REQUIRE_SIGNED_COMMANDS is documented but has nowhere to actually be set
- HEARTBEAT.md, TOOLS.md, USER.md are committed in every agent workspace but undocumented
