RegisterDateTimeWithTimeZone#initialize_type_map

Run original `initialize_type_map` and then register `timestamp` as a
`MysqlDateTimeWithTimeZone`.

When schema dumping, `timestamp` columns will be output as
`t.datetime_with_timezone`.
