Parser::Diagnostic::Engine#process

Processes a `diagnostic`:
  * Passes the diagnostic to the consumer, if it's not a warning when
    `ignore_warnings` is set.
  * After that, raises {Parser::SyntaxError} when `all_errors_are_fatal`
    is set to true.
