- Decompiled call definitions have no name, so beam-only protocols and `__using__` are never recognised
- Synchronous VFS refresh under a read lock in buildExternalLibraryPlans
- Mix deps sync queries the workspace file index before it is initialized, silently dropping a module
- SdkRegistrar dedupes by home path but the workspace model keys on (name, type), so registration can throw SymbolicIdAlreadyExists
- depsStatus catches the wrong ExecutionException, so a cancelled mix deps becomes a crash report
