- A stale branch silently reverts main's citation re-anchor on merge, and --apply cannot fix it
- Windows CI runs only 2 portability files; 14 suite tests fail on a real Windows host
- A manually landed task is resumed on pr_conflict and cannot be reconciled with --landed
