- app-server: silent exit 0 mid-turn immediately after a shell tool call, in read-only sandbox (macOS)
- [Windows][Android Remote] Individual 2–5 MB tasks hang on loading skeletons while a 1 MB task opens
- Codex App: infinite sign-in loop — app never receives a chatgpt.com session cookie, so backend-api calls 401 ("Missing valid access token or actor biscuit")
- Session becomes unusable: Invalid input[N].id = fc_... (expected ctc) when replaying history
- After the update, I lost my projects and servers
- Android: Voice Mode hides connected app tools; switching to text exposes them in the same chat
