- js_printer: fold adjacent member reads into a pattern only when minifying a bundle

- shell: exit 126 for a path that is there and cannot be executed
- shell(rm): take the operands in command-line order from one pool task
- js_parser: keep `typeof name` and the lookups of a body-declared `var` inside `with`
