- Parked-session surfaces can never report a parked session: wire the writer and separate empty from unavailable
- A recovery path that cannot fire looks like one that was never needed: record guard evaluations per run
- The orchestrator's own prompt bytes are unrecorded: segment and digest them at assembly
- Operator review comments never enter the record: anchor each annotation to the diff bytes it targets
- Injected skill set for a spawn is recorded only in an opt-out local log, never in the run record
- What an agent was shown is unrecorded: derive a content-addressed context manifest per task and anchor it to the run
- Context-window fill has measurement but no enforcement: give it the budget ladder every other resource already has
- Stall kills discard the state the resume surfaces were built to consume: write the checkpoint at the kill boundary
- Verification reads the diff but never probes it: derive a deterministic boundary-input gate from the changed surface
