- LifecycleEvent.POST_MERGE is declared and subscribable but never dispatched on merge-back
- quarantine: transient host resource exhaustion records permanent cross-run skip quarantine
- spawn_prompt: prompt templates hardcode literal port 8052 in curl examples
- adapters: pi and claude workers inherit user global MCP servers without isolation flags
- worktree: copy_files into agent worktree without git exclude causes merge conflict on commit
- orchestrator: _had_any_terminal_task checks only done/failed and stalls quiescence on closed tasks
- Cost-anomaly stop-spawning signal and per-task checks are not wired into the orchestrator lifecycle

- audit_pack tail-digest never sees a non-dict JSONL line under test, so losing the isinstance guard would crash or mis-quote the evidence pack unnoticed
- _run_opa_eval error path is never executed under test, so a regression that swallows opa's stderr would ship green
