- ReplicatedMergeTree: ALTER UPDATE accepted on a replica with stale metadata after a concurrent RENAME COLUMN; mutation fails forever with NO_SUCH_COLUMN_IN_TABLE and wedges the table's mutation queue

- Restore lazy materialization over a subquery when a row policy is the only filter
